The Care Quality Commission (CQC) rated Windmill Care Home inadequate following an inspection which was triggered by an incident involving a woman in her 90s.
Inspectors raised concerns about safeguarding arrangements and whether residents were being adequately protected from potential harm.
Being placed in special measures means the home will now face increased scrutiny from the regulator and will be expected to make significant improvements within a set period.
The CQC can take further enforcement action if sufficient progress is not made, while inspectors are expected to return to assess whether standards have improved.
The care home says it accepts that changes are needed and has begun taking significant steps to address the concerns raised in the inspection.
Managers say work is under way to strengthen procedures, improve staff practices and ensure residents receive safe and appropriate care.
The CQC will continue to monitor the home while the improvement programme is carried out, with the future rating dependent on progress made before the next inspection.
